Home Care Nurse


GENERAL SUMMARY: Responsible and accountable for the clinical operational efficiencies and the quality healthcare outcomes through effective leadership and management of department staff. Consults with and advises management to ensure that department goals and performance measures are met. PRINCIPLE DUTIES: Client Care 1. If needed by state law, completes an initial assessment of client and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Regularly re-evaluates client nursing needs. 3. Initiates the plan of care and makes necessary revisions as client status and needs change. 4. Develops a care plan and includes the client and the family in the planning process. 5. Administers medications and treatments as prescribed by the physician. Provides direct client care as defined in the Florida Nurse Practice Act. 6. Counsels the client and family in meeting client needs. 7. Provides health care instructions to the client as appropriate per assessment and plan of care. 8. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the client. Communication 1. Prepares clinical notes and updates the primary physician when necessary. 2. Communicates with the physician regarding the client's needs and reports any changes in the client's condition; obtains/receives physician's orders as required. 3. Communicates with Agency health related persons to coordinate the care plan. Additional Duties 1. Participates in on-call duties as defined by the on-call policy. 2. Instructs, supervises and evaluates paraprofessional care provided every 60 days or as required by state law. QUALIFICATIONS: 1. Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting. 2. Current licensure in the state and CPR certification. 3. Management experience not required. Responsible for supervising personnel care assistants. 4. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. 5. Prolonged or considerable walking or standing. Able to lift, position or transfer clients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills. 6. Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements. PREFERENCES: 1. LPN (Required) 2. RN (Preferred) PHYSICAL REQUIREMENTS/WORKING CONDITIONS: Frequent standing, stooping, bending, stretching, squatting; may be exposed to blood and body fluids which may contain HIV and/or HBV; must be able to transport residents via wheelchair, gerichair or shower chair, pushing up to 150 lbs. unassisted. Must be able to transfer, lift, turn and position a resident weighing up to 125 lbs. unassisted, or with the assistance of one for a resident weighing over 125 lbs. Must be able to stoop, bend, stretch, squat, stand, and walk for up to 90% of the workday. May be subjected to offensive odors and combative behavior. Must be able to reach up to a level of six feet; must be able to push a dietary cart up to 40 lbs. unassisted. Must be able to lift and carry up to 20 lbs. frequently, and 50 lbs. occasionally.
