By agreeing to participate, you agree to the following Program terms and conditions:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">The Program is offered to certain healthcare professionals who have previously been placed at health systems or with agencies via Vivian\u2019s job portal. Criteria for selection of participants will be determined by Vivian in its sole discretion and may change from time to time.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Eligibility for the Program is reserved to invited individuals who have agreed to participate and have agreed to these rules.\u00a0 Invitations to participate in the Program are unique to you and may not be shared with or forwarded to any other individual.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Prior Job Placement Information.\u00a0 Unless Vivian already has your prior job placement information in its records, you will be requested to provide your prior employment information for the last job you found via the Vivian job portal in order to be eligible for the Program. All additional information collected by Vivian to administer the Program shall be processed in accordance with Vivian\u2019s Privacy Policy.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Next Job Reward.\u00a0 Eligible participants will receive a $100 gift card (\u201cReward\u201d) when they confirm to Vivian that they have found their next job at a facility or placed by a recruiter via Vivian\u2019s job portal. Eligible participants are limited to one Reward per month.\u00a0 Rewards will be delivered within ___ days of the end of the month during which the participant found their next job via the Vivian portal and such placement has been confirmed by Vivian.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Vivian reserves the right to request verification of qualification criteria set forth above, to verify that reporting by the eligible participant in their next job is truthful and accurate.\u00a0 To enable such verification, Sponsor may request any supporting documentation including screenshots of evidence of employment during the relevant calendar month or other documentation as reasonably requested by Vivian.\u00a0\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Rewards will be shipped to Participant\u2019s email address or residence address as provided by Participant to Vivian or to Vivian\u2019s rewards fulfillment vendor.\u00a0\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Rewards are not redeemable for cash, alternative gift cards or e-gift cards. Rewards are not valid on tax or shipping, and cannot be combined with other offers or promotions. As a Participant, you are responsible for any and all tax liability resulting from receiving a Reward.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Subject to applicable laws, Vivian makes no warranties, and hereby disclaims any and all warranties, express or implied, concerning any prize furnished in connection with the Program.
