Nurse Practitioner (Bronx, NY, Us)

POSITION SUMMARY: Patients with tracheostomy are often critically ill so relatively small tracheostomy-related adverse events can have a tremendous impact on patient morbidity and outcomes. At Montefiore currently there is no health care provider who can administer regular care to inpatients with tracheostomies. From discussions in airway committee meetings involving high level nurses, respiratory therapists, physicians and administrators it is clear that nursing and respiratory therapy do not currently have the staffing to provide or hire a person or group of people to routinely care for and evaluate tracheostomy patients. To meet this gap in patient care we propose the hiring of a nurse Practioner to round on all in-patients with tracheostomies, to serve as a liaison with other health care providers and improve tracheostomy related education, and to see these and other tracheostomy patients as outpatients. Purpose of the Position: * Provide daily tracheostomy care to all inpatients * Work as a tracheostomy care educator for inpatient health care providers * Improve outpatient tracheostomy care via a weekly tracheostomy clinic * Improve the outcomes of patients with tracheostomy * Decrease tracheostomy related adverse events EDUCATION, TRAINING, LICENSES, AND WORK EXPERIENCE: * Master's Degree or Doctorate from nationally accredited Nurse Practitioner educational program. * DEA certificate. * Unrestricted license to practice in state and per Physician/APN Standard Care Arrangement. * Minimum of 5 years nursing experience, including care of patients with complex, chronic and/or life-threatening conditions, and those at end of life; clinical nursing care in oncology, hospice, intensive care preferred * Hospice and Palliative Care Certification or eligibility desirable. * Knowledge of medications and prescriptive authority per New York state law. * Knowledge of Spanish preferred.
