Requisition No: 592526 Agency: Agency for Persons with Disabilities Working Title: SENIOR LICENSED PRACTICAL NURSE - 67052770 Position Number: 67052770 Salary: $45,810.79 to $55,431.06 Posting Closing Date: 12/30/2022 SENIOR LICENSED PRACTICAL NURSE APD TACACHALE GAINESVILLE FLORIDA This is work providing practical nursing services to developmentally disabled individuals at Tacachale. Work is performed under the direction and general supervision of a registered nurse, and is reviewed for compliance, standards and procedures. Work Schedule: These positions are located at a 24- hour facility. 1st shift from 6:00 a.m. to 2:30 p.m., with rotating days off 2nd shift from 2:00 p.m. to 10:30 p.m., with rotating days off 3rd shift from 10:15 p.m. to 6:15 a.m., with rotating days off Nursing: Prepares, administers and documents medications and treatments as per nursing policy and procedure. Maintains documentation in accordance with established policies and procedures. Observes, records and reports physical and behavioral response to medication and treatment. Uses nursing process for ongoing care of individuals as need is identified. Intervention would include, but not be limited to; observation, referral to the registered nurse, medication and treatment as prescribed and/or crisis intervention. Maintains work area, medical supplies and equipment, and adheres to infection control policies. Accompanies individuals to clinic and off-center appointments as assigned. Provides emergency nursing service within the scope of nursing practice. Provides nursing care and administers treatment as directed by a registered nurse to subacute, chronic, and convalescent residents. Assists a registered nurse and/or a Licensed Independent Practitioner (LIP) in clinics, diagnostic tests and physical examinations by preparing clinic room for use, collecting routine specimens, taking and recording temperature, pulse, respiration and blood pressure. Prepares and administers hypodermic injections as ordered by the attending LIP. Performs simple rehabilitative nursing procedures for selected chronically ill and aged residents. Assists a registered nurse with screening procedures. Assists LIP with physical examinations. Assists registered nurses in completing and maintaining medical records and residents' progress. Incident Management: Conduct an initial assessment and provide appropriate treatment as soon as possible after being notified or becoming aware of an injury or possible injury. Complete Section II of the DDC Incident Report Form by providing a detailed description of the injury (size, shape, color, location on the body). Indicate the treatment given and the plan of care. Establish the Type of Incident and Level of Injury. Take two (2) sets of photos and complete the Photographic Wound Documentation form. Ensure the Florida Abuse Hotline is contacted if abuse, neglect, exploitation, mishandling, mistreatment or sexual abuse/activity is witnessed, known or suspected. Forward the Incident Report Form to the supervisor/in-charge staff for further action. As warranted, comply and assist authorized investigators from APD and other agencies, such as law enforcement, DCF Adult Protective Services, Medicaid Fraud Unit, and the Agency for Healthcare Administration. Administrative: Assists in the development and monitoring of nursing service objectives. Participates as appropriate in all objectives developed for the individuals assigned. Works in conjunction with the registered nurse to assure continuity of care and adherence to ICF/DD standards. Attends interdisciplinary team meetings and special clinics as assigned. Participates in and reinforces health care teaching to assure adherence to current principles of care as ordered. Attends in-service training and participates in continuing education programs for the developmentally disabled person. Monitors and role model's infection control programs during daily activities. Adheres to ethical practice standards. Participates in nursing committees as assigned. Performs other related duties. Knowledge, Skills, and Abilities: Knowledge of nursing principles, practices and techniques. Knowledge of human anatomy and physiology. Skilled in the use of emergency equipment within scope of training. Ability to interact, assess and respond to individuals who are developmentally disabled. Ability to provide emergency nursing care to include but not limited to basic life support, emergency care for choking (abdominal thrust), rescue breathing and use emergency equipment. Ability to stand, walk, sit and bend for periods in excess of three hours. Ability to assist in lifting and transferring residents weighing more than 100 pounds, as needed. Ability to successfully complete Prevention and Safety Strategies Training. Minimum Requirement(s): A valid Florida Driver's License. Current Practical Nurse License & CPR Healthcare Provider Certification.
