CLINICAL CARE MANAGER (RN)

FULL TIME - 8 HOURS SHIFT - FOR THE NAVY TEAM

SERVING HOMESTEAD, FLORIDA


Summary & Objective

The Director of Clinical Care is responsible for administration of patient care services. The Director of Clinical Care plans, implements and directs patient care services to assure quality care and appropriate quantity of services.


Essential Functions

  • Ensures implementation and adherence to Hospice Program standards, Medicare Conditions of Participation and systems pertaining to clinical operations.
  • Ensures organizational growth through best practices, regulatory compliance, quality improvement and staff development resulting in patients and their families receiving quality care and coordination of hospice services in a manner that meets their needs.
  • Determines and obtains appropriate staffing levels for services with the goal of providing consistent quality care for patients.
  • Develops budgets with the appropriate managerial staff.
  • Develops and monitors the budgets for patient care teams.
  • Forecasts patient activity and acuity for the following fiscal year.
  • Develops, manages, and supports performance improvement activities.
  • Collaborates and participates in committees such as Pharmacy & Therapeutics, Ethics, or others as required.
  • Serves as a liaison to the community in marketing product service and participates in organizational, marketing and community activities to promote organizational growth.

Other Duties

  • Directly supervises several Clinical Care Managers to insure compliance with services, related to staffing and services.
  • Promotes patient safety and performance improvement, directs and coordinates nursing and home health aide services.
  • Ensures all staff is in compliance with state health statutes and administrative rules concerning guidelines for safety, universal precautions and infection control.
  • Works collaboratively with Admissions, Community Relations, and Interdisciplinary Team Managers and members to develop and integrate process improvement.
  • Responsible for on call emergencies, including weekends and holidays.
  • Promotes positive employee engagement, teamwork, mutual respect and safety work practices.
  • Maintain required mandatory skill updates.
  • Comply with all policies, local, state and federal laws and regulations.
  • Perform other duties as assigned.

Supervisory Responsibility

  • May serve as an interim department leader depending on need

Physical Requirements

  • Must be able to lift and/or move up to 50 pounds and push/pull up to 250+ pounds, walk, climb stair or ladders, stand on feet for extended periods of time, etc.

Requirements

Knowledge & Experience Requirements

  • Bachelor’s Degree in Nursing required; Master’s degree preferred.
  • Clear and Active Registered Nurse license to practice in the state of Florida.
  • 3-5 years management experience preferred in healthcare field.
  • Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.)
  • Must have knowledge of computer office/clinical software.
  • Must be able to read, write and understand the English language.
