Founded in 1950 in Anahuac, Texas, with a hospital along Trinity Bay, the town still serves as the flagship location and headquarters for the health system. It is home to the hospital, a primary care facility, dental services, transitional care services and a community center that has a full fitness facility, theater, event space, heated pool, physical therapy and offices for rent by non-profits or small businesses. OmniPoint Health’s primary care facilities receive program grants as part of its designation as a Federally Qualified Health Center (FQHC), which are community-based healthcare facilities that provide comprehensive primary care and preventive care, including medical, oral and mental health/substance abuse programs and services for all residents in the community.
