Description

Hours: 8am to 4:30pm (40 hours/week)
Coverage area: Arlington, Everett, Malden, Medford, West Medford, Melrose, Somerville, MA
Requirements:
• Minimum 1 year of experience in certified home care
• Minimum 1 year of case management experience
• Ability to travel to assigned patient locations
• Weekend rotation required every 6 weeks
Job Summary:
We are seeking a dedicated and compassionate Home Health RN to join our team. The ideal candidate will be responsible for assessing patient conditions, developing and implementing individualized care plans, and providing ongoing education and support to patients and their families.
Responsibilities include:
• Delivering patient care in accordance with organizational philosophy, policies, and community health standards
• Performing patient admissions and case management functions
• Maintaining thorough, timely, and accurate electronic documentation
• Collaborating effectively with interdisciplinary teams to ensure high-quality care
