POSITION SUMMARY:
The Clinical Intake Specialist is responsible for the timely and accurate completion of the referral process in coordination with the Executive Director (ED) and/or Director of Patient Care Services (DPCS) and Sales team of the agency location. The Clinical Intake Specialist is responsible for the timely acceptance or declination of the referral, all coordination of the referral information to clients, their family members, physicians, referral sources, facility staff, agency staff, and any others involved in assuring continuity of client care. The Clinical Intake Specialist services as a focal point for coordination and communication for patients, family members, physicians, referral sources, Shared Services, and agency staff.
EXPERIENCE/TRAINING/EDUCATION/CERTIFICATION:
- Current state RN, preferred
- Minimum of one year experience in home care, hospice, or personal care services, preferred
- Prior use of Electronic Medical Records (EMR), preferred
- Experience in dealing with a variety of payers in healthcare, e.g., Medicare, Medicaid, and commercial payers, preferred.
SKILLS/ABILITIES:
- Customer service focused, including the ability to coordinate and communicate effectively and efficiently
- Ability to work with a variety of team members representing other functional areas that directly or indirectly impact client Intake
- Ability to maintain composure in addressing stressful and/or challenging situations
- Excellent telephone, verbal and written communication skills, with the ability to communicate with both professionals and non-professionals
- Excellent organizational skills with attention to detail.
- Knowledge and skills in the use of computers and software programs
- Knowledge of pertinent state, federal, and local regulatory requirements
Shifts: These are 8/10/or 12 hour shifts that will include the Weekends. Shift is set depending upon placement. We do cover East & West Coast.
