Required Certifications & Verifications
- ACLS (Advanced Cardiac Life Support) - AHA or Canadian equivalent
- BLS (Basic Life Support) - AHA or Canadian equivalent
- PALS (Pediatric Advanced Life Support) - AHA or Canadian equivalent
- NRP (Neonatal Resuscitation Procedure) - Current certification by AAP, AHA, or Canadian Paediatric Society
- Chemotherapy Certification (if applicable)
- Electronic Fetal Monitoring (EFM) Certification (if applicable)
- National Tech Certification Verification (as applicable)
- State License Verification (Oregon pending license acceptable)
- NURSYS Verification (for RN, LPN/LVN, AP)
- IV Certification (if applicable)
- Management of Assaultive Behavior (MAB) (if applicable)
- NIH Stroke Certification (if applicable)
- CPI (Crisis Prevention Institute) Certification (if applicable)
Competencies
- Medication Test/Pharmacology Competency Exam (as applicable)
- Color Blind Testing (unit-specific, if required)
- Competency Exams relevant to profession/specialty
- Fetal Heart Rate Monitoring Assessment or EFM Certification for L&D RNs (effective 9/30/2024)
- Medication Aide Exam for CNAs in Oregon Assisted Living facilities (if applicable)
Health & Safety Requirements
- Physical Exam within 1 year of start (signed by MD, NP, PA, or DO)
- Fit Mask Test (N95 or equivalent) - Required for travel/new starts after 2/5/2024
- Tetanus/Diphtheria/Tdap Vaccine (within 10 years) or declination
- Negative TB test or Chest X-Ray within 12 months, updated annually
- MMR Vaccines or Positive Titers for Rubeola, Mumps, Rubella
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- 10 Panel Drug Screen within 90 days of start (no instant tests)
- Seasonal Flu Vaccine or declination (required during flu season)
- COVID-19 Vaccine or declination (required if onsite)
- Respiratory Safety Clearance (RespSafety) for travel/new starts after 2/5/2024
Background & Employment History
- Background Check within 45 days of start, covering last 7 years
- EPLS/GSA/SAM checks within 30 days of start, updated monthly
- Minimum 2 references covering 1 year of employment in a comparable specialty/setting (6 months for CNAs)
- Employment Application covering at least last 5 years (7 years for APN/Subcontractors)
- Skills Checklists (annual updates as applicable)
- Education Verification (3rd party) for all candidates
- Radiation Badge issued by facility if applicable
Facility Specific Requirements
- Must complete Providence Oregon Floating Policy Acknowledgement (acknowledges possible float to other Providence facilities nearby)
- Completion of required WorkRamp training modules prior to start (Core, Specialty, Regional, and Annual Regulatory modules as applicable)
- Epic training modules required for charting system access
- Photo for badge (passport style with neutral background) required
Licensing
- Oregon pending license accepted
- Washington multi-state licensed nurses must complete demographic attestations prior to start (if applicable)
Notes
- No weekends or on-call required
- Block scheduling required for night shifts Wed-Sat
- No 48-hour shifts approved
- Parking is free at the facility
- Epic charting system used
