Description
Title: Licensed Medical Social Worker (LMSW) – Hospice
Specialty: Hospice Care
Schedule: Monday – Friday
Hours: 25 patient-facing hours weekly / 40 hours per week
Location: New Hampshire (Territory-based)
Position Overview We are seeking an experienced Licensed Medical Social Worker (LMSW) with recent hospice experience to provide compassionate support and care coordination for geriatric patients in a home hospice setting. The ideal candidate will be comfortable traveling throughout the assigned territory, maintaining timely documentation, and collaborating with the interdisciplinary care team.
Schedule & Caseload- Monday – Friday
- 40 hours per week
- Approximately 5–7 patient visits per day (please confirm during interview)
- No weekends
- No on-call requirements
- Active New Hampshire LMSW license in hand
- Minimum 1 year of recent Hospice Social Work experience
- Experience with HCHB (Homecare Homebase) EMR required
- BLS Certification required
- Strong documentation and communication skills
- Must be flexible with assigned territory coverage
- Primarily Geriatric Hospice Patients
- Coverage area may extend up to a 60-mile radius from the assigned office/hub
- Traveler must be willing to float within the designated territory as needed
