There is also apsychiatric nurse practitioner in the clinic toassist with medication management. Harm Reductioncounselors who are licensed chemical dependencycounselors assist patients at high risk forsubstance use/misuse.(4) Oncology Rehabilitation is a resource toassist persons with cancer enhance their physicalfunction and mobility that may have been impaireddue to cancer and/or treatment. A physician offersinterventional pain procedures as well as assistpatients with musculo-skeletal functioning. Aphysical and occupational therapist evaluate andtreat patients by referral.(5) Survivorship care is designed to promotewellness and care coordination for cancersurvivors. NPs are the providers who complete acomprehensive needs assessment and facilitate aplan of care that addresses late and long-termeffects of cancer treatment. Additionally, forpatients who complete active treatment for curativeintent receive a treatment summary & survivorshipcare plan that is a road map of what is to come.Patients receive this within 0-6 months offinishing treatment.7.0 Adolescent and Young Adult services forpatients diagnosed with cancer between the ages of15-39 yrs old. Services include consultation withAYA social worker for supportive needs assessmentand resources.
