This position involves 8-hour day shifts with no weekend or on-call requirements.

Job Description

The Occupational Therapist will be responsible for effectively screening and evaluating patients with cognitive and/or Activities of Daily Living (ADL) physical functioning disorders. The role involves developing suitable plans of care in compliance with regulatory and clinical practice standards. Key responsibilities include:

  • Utilizing standardized assessments and evidence-based practices to support clinical interventions.
  • Providing rehabilitative, skilled, and medically necessary treatment interventions for patients with cognitive and/or ADL physical functioning disorders, adhering to all regulatory and clinical practice requirements.
  • Administering various rehabilitation procedures, including but not limited to, ADL retraining, therapeutic activities, therapeutic exercises, and the use of supportive and assistive devices.
  • Assessing patient progress and adjusting treatment plans accordingly, while maintaining comprehensive and accurate clinical records.
  • Evaluating treatment outcomes for effectiveness and modifying services as needed based on patient evaluations.
  • Offering consultations and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
  • Communicating effectively with supervisors and other healthcare team members about patient progress, barriers, and treatment plans.
  • Providing a detailed discharge summary of services rendered, meeting all regulatory and clinical requirements.
  • Documenting all necessary information in the electronic documentation system and maintaining patient records according to company policy.
  • Recording treatment minutes accurately in compliance with all regulatory and clinical requirements.
  • Participating in required meetings, including patient care conferences, utilization review meetings, and rehabilitation meetings.
  • Engaging in required facility and company trainings, in-services, and conferences.
  • Obtaining necessary durable medical equipment (DME) or supplies as per regulatory and clinical practice requirements.
  • Supervising and supporting occupational therapist assistants, rehabilitation aides, and/or students as needed.
  • Adhering to NBCOT and state practice acts, as well as the code of conduct and ethical standards during professional practice as an Occupational Therapist.

Qualifications

  • Minimum of 1 year of occupational therapy experience.
  • Experience in long-term care or rehabilitation settings is required.
  • Must possess an active occupational therapist license.
  • New graduates will be considered if they have completed Level II experience.
