Job Opportunity

Position: Physical Therapist (PT)

Specialty: Long Term Care (LTC)

Shift Details:

  • 8-hour shifts
  • 5 shifts per week
  • No weekend requirements

Job Description

As a Physical Therapist, you will be responsible for effectively screening, evaluating, and rehabilitating patients with physical functioning disorders and/or wound care. You will develop appropriate plans of care while adhering to all regulatory and clinical practice standards. Your key responsibilities include:

  • Utilizing standardized assessments and evidence-based practice to inform clinical interventions.
  • Administering various rehabilitation procedures including manual techniques, ambulation, therapeutic exercises, modalities, and wound care.
  • Assessing patients’ progress and adjusting treatments accordingly in compliance with regulatory and clinical requirements.
  • Evaluating treatment outcomes for effectiveness, modifying services as necessary based on patient evaluations, and making referrals when appropriate.
  • Providing consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
  • Effectively communicating with supervisors and healthcare team members about patient progress, barriers, and treatment plans.
  • Preparing comprehensive discharge summaries of services rendered in accordance with regulatory and clinical standards.
  • Documenting all required information in the electronic documentation system and maintaining patient records per company policy.
  • Recording treatment minutes accurately to meet regulatory and clinical standards.
  • Participating in mandatory meetings such as patient care conferences, utilization review meetings, and rehabilitation meetings.
  • Engaging in required trainings, in-services, and conferences.
  • Coordinating necessary durable medical equipment (DME) or supplies in line with regulatory and clinical requirements.
  • Overseeing and supporting physical therapist assistants, rehabilitation aides, and/or students.
  • Adhering to the APTA and State Required practice acts, code of conduct, and ethics during professional practice.

Requirements:

  • 1 year of Physical Therapy experience is required.
  • Long Term Care or Rehabilitation experience is required.
  • Current Oklahoma State License is required.
