Outpatient Occupational Therapist - Travel/Contract
Location: Flagstaff, Arizona
Contract Length: 13 weeks
Job Details
- Setting: Outpatient therapy clinic with Occupational Therapy (OT), Physical Therapy (PT), and Speech-Language Pathology (SLP) services
- Caseload: 8-11 patients per 8-hour day; approximately 11 patients per 10-hour day
- Shift Options:
- 4 days per week, 10-hour shifts (8:00 AM - 6:00 PM)
- 5 days per week, 8-hour shifts (8:00 AM - 5:00 PM)
- Schedule: Monday through Friday, blocked scheduling
- Patient Care: 1:1 direct care
- Multidisciplinary collaboration with orthopedics, neurology, and primary care providers
- Weekend, on-call, and floating assignments are not required
- Locals accepted
Required Qualifications
- Valid Occupational Therapist license in Arizona
- Minimum 1 year of Occupational Therapist experience within the last year
- Basic Life Support (BLS) certification
Work Hours & Expectations
- Guaranteed 40 hours per week
- Facility may call off up to 6 times during a 13-week assignment
- Business casual dress code
- Charting system: Cerner
- Free parking available
- Adherence to facility quality and patient safety standards, including medication administration protocols (BCMA)
Compliance & Documentation Requirements
- Employment application covering last 3 years with documented gaps over 30 days
- Two professional references required (one from most recent employer within 1 year)
- Background checks including 7-year criminal history, EPLS/GSA/SAM, HHS/OIG within 30 days of start
- Education verification (diploma or transcripts) required
- Physical exam within 1 year confirming ability to perform essential job functions
- Drug screening (10-panel) within 30 days of start and updated annually
- Immunizations and health requirements:
- Tetanus/Diphtheria/Tdap (preferred within 10 years or declination)
- Negative TB test or chest X-ray within 1 year, updated annually
- Measles, Mumps, Rubella, Varicella immunity (positive titer or vaccination proof)
- Hepatitis B immunity or declination
- Seasonal flu vaccination or facility-specific declination form (mask required if declined)
- COVID vaccination or declination with facility-specific form
- Fit mask (N95) required within 1 year of start and updated annually; facility may provide onsite fit testing if needed
- Completion of facility-specific job description and policies prior to start
- Security question form and travel information verification must be completed and signed before start
- Photo for badge required (clear headshot with preferred background)
Additional Notes
- No weekend or on-call duties expected
- No floating to sister facilities required
- RTO (request time off) must be approved by the facility
- Opportunity to work in a multidisciplinary outpatient clinic environment
