Monitor and evaluate the psychosocial and discharge planning needs of patients in both the medical and psychiatric setting.
Assist patients and families in resolving personal/social problems interfering with the treatment process
Educate patients and families to increase understanding and effective use of medical and psychiatric services.
Coordinate services to facilitate timely patient discharges
Act as a resource to patients, families, and other health care team member
Develop and maintain knowledge and understanding of community and hospital resources
Provide education and support to patient/family and assist in formulating a safe individualized plan of care
Work collaboratively with patients, families, hospital staff, physicians, and other individuals and agencies involved in providing patient care
Complete written documentation in an accurate, concise, and timely manner
Coordinate tasks and workload responsibilities in an organized and efficient manner
Conduct work in accordance with hospital time frames
Coordinate financial assistance for patients based on need
Coordinate complete patient care needs with the interdisciplinary team
Assist with population specific needs as appropriate
Masters prepared social workers will additionally be responsible for providing individual therapy, group therapy, and clinical guidance of social service staff
Performs other duties as assigned

Graduate from an accredited school of Social Work, a Bachelor’s Degree in Social Work (BSW) is required. Masters Degree in Social Work is preferred.
Minimum of five years of social work experience is preferred
Experience in healthcare is also preferred
Excellent verbal and written communication skills
Must be able to read, write and speak English
