• Developing individualized patient care plans
• Coordinating care with physicians & healthcare teams
• Performing patient assessments & re-evaluations
• Educating patients & caregivers on self-care & medications
• Monitoring patient progress & documenting outcomes
• Supporting patients through compassionate home health care

✅ Requirements:
• Active Vermont & New Hampshire RN License OR Compact License
• Minimum 2 Years of Home Health Experience
• Graduate of an Accredited Nursing Program
• BLS Required

CloudinfraIT Inc Job ID #296712. Pay package is based on 12 hour shifts and 36 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
