Description

Nurse Administrator of Care Management.This is a newly created position for the overall planning, direction and implementation of roles, processes and programs including: Case Management and Social Work Services.

- Transitions of Care
- Outcomes Management"
Skills: "Additional qualifications Required
- 5+ years' experience in progressive management positions
- 5+ years' experience in an inpatient hospital Care Management department in an RN/MSW integrated model
- Minimum of five+ years in progressive leadership responsibility
- Minimum of three years care management/coordination experience in an acute care – inpatient hospital care environment.
- Exhibits excellent communication skills, both verbal and written, and does so with high quality and integrity.
- Demonstrates the ability to be flexible and adaptable in a highly stressful and constantly changing health care environment.
- Previous experience in large and complex health care systems
- Previous experience in establishing or significant enhansement care management models in a health system or facility
