Candidate Requirements

• Residency: Candidates with an Arizona home address on their driver’s license will NOT be considered. A copy of the driver’s license must be included in the submission.
• Experience: Minimum 24 months of current Level I Trauma RRT experience (required).
• Certifications:

  • Must have active American Heart Association (AHA) or American Red Cross certification at time of submission (required).
  • BLS (AHA) and NBRC certifications required.
• Licensing:

  • Active Arizona Respiratory Care Practitioner License required.
  • No license hits allowed.
  • Compact license allowed.
• Vaccination: Flu vaccination required; declinations accepted with proper forms. Vaccination status must be noted in submission.
• Background: Must meet Banner’s specific background compliance requirements (attestation required).
• Preferred Experience: PICU/NICU and travel experience preferred.

Submission Requirements

• Fully completed Banner Cover Page.
• Resume detailing all facilities worked at, including facility size, bed count, trauma level, and units.
• Skills checklist specific to respiratory therapy, dated within the last year.
• Active license verification dated within the last 30 days.
• Current certifications included in submission.
• Driver’s license copy included.
• All requested departments and shifts the candidate is willing to work must be clearly listed.
• Ready to accept assignment upon submission (preclosed candidates only).

Assignment Details

• Start date should be on or very soon after the order start date.
• Return to Office (RTO) requests:

  • Must be less than 7 days.
  • Not approved during the first week of assignment or on holidays.
• Learning modules must be completed prior to assignment start.
• Non-billable hours adjusted to 16 hours per assignment.
• Assignments may be canceled with 7 days written notice without penalty.
