Radiology Technician (Rad Tech) - Long-Term Assignment
Location: Davenport, Iowa
Contract Length: 13 weeks
Shift: 8:30 AM – 5:00 PM
Required Qualifications
- Valid Radiology Technician license in Iowa
- Minimum of 1 year of Radiology Technician experience within the last year
- BLS certification issued by the American Heart Association
Onboarding and Compliance Requirements
Candidates must provide the following documentation, all to be current and valid as specified:
- License/Certificate Verification:
- Online verification dated within 30 days of start.
- Health and Safety:
- Fit Test
- 10-panel drug screen completed within 30 days of start (no THC)
- COVID-19 vaccine proof including booster or approved declination/exemption (declination not accepted in CA or IL)
- MMR: Proof of 2 vaccinations or IgG titer lab results
- Hepatitis B: At least first vaccination in series prior to start (titers not accepted)
- Influenza vaccine (optional)
- 2 Step PPD or IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
- Varicella: Proof of vaccination or IgG titer
- Physical/Health Statement confirming fitness to work within the last year
- Current, non-expired Tdap vaccine (declination accepted if no pediatric interaction)
- Professional and Background Checks:
- Criminal background check (federal, nationwide, county) from last 7 years, dated within 30 days of start
- Social Security Number verification
- Education verification (highest academic degree or diploma/GED)
- EPLS (Excluded Parties List System) check dated within 30 days
- OIG (Office of Inspector General) exclusion check dated within 30 days
- NSO (National Sex Offender) check dated within 30 days, including aliases and locations
- Iowa-specific Child and Adult Dependent Abuse Training and registry checks (criminal history, sex offender, dependent adult & child abuse registries) dated within 30 days
- OFAC (Office of Foreign Assets Control) sanctions check within 30 days
- Competency and Skills:
- Specialty-specific competency testing completed within the last year
- Skills checklist provided by agency for clinical contractors
- CORE exam (Joint Commission testing) within the last year
- Forms and Agreements:
- Signed Code of Conduct form
- Signed Confidentiality of Patient and Proprietary Information and Information System Security Agreement
- Signed Integrity and Audit Services form
- Completed Non-Employee Upload form (pages 1-3 and 28)
- Conflict of Interest Disclosure form
- System Access form completed and uploaded one week before start
- Completed profile including resume, skills checklist (within a year), and two professional references
- Completion of facility-specific PowerPoint and signature page for Iowa locations
