Job Description
We are seeking a compassionate and skilled Home Health Registered Nurse (RN) to provide exceptional care to our patients in a home setting. The ideal candidate will be responsible for executing personalized patient care plans, supporting patients' health needs, and ensuring the highest quality of care.
Responsibilities
- Conduct thorough assessments of patients' physical, emotional, and psychological needs.
- Develop and implement comprehensive care plans in collaboration with the healthcare team.
- Administer medications and monitor patient progress.
- Educate patients and families about health management and safety precautions.
- Utilize various medical equipment and devices necessary for patient care.
- Document patient interactions and care provided accurately.
- Perform skilled home care visits, ensuring necessary documentation and billing accuracy.
Required Experience
- Must have at least 1 year of home health experience.
- OASIS training preferred.
- Experience with various patient conditions including med/surg, cardiac, orthopedic, and respiratory.
Certifications & Licensure
- Active RN license in the state or a compact license.
- Current Basic Life Support (BLS) certification.
