Job Opportunity: RN Case Management / Discharge Planning

Job Summary

Seeking highly experienced Registered Nurses with acute inpatient case management and discharge planning expertise. Minimal training provided; candidates must quickly adapt and work efficiently.

Contract Length

• 13 weeks

Schedule

• Monday to Friday, 8:00 AM – 4:30 PM
• 8-hour shifts, 40 hours per week
• No weekend requirements
• Holiday shifts rotated as needed

Patient Load

• Manage 15-20 patients or consults per day
• Coverage across all inpatient units in the hospital

Required Qualifications

• Valid RN license in Mississippi
• Minimum 3 years of case management experience
• Case management experience within the last year
• BLS certification issued by the American Heart Association
• Associate of Science in Nursing / Associate Degree in Nursing
• Experience in an inpatient setting
• Acute hospital case management experience
• Discharge planning experience
• Experience with Epic EMR

Additional Requirements

• Flu vaccine required (medical exemption allowed with documented evidence)
• Fingerprint background check required (remote or in-person options available)
• Must be able to work within a 50-mile radius

Orientation & Team

• 2-week department-specific orientation including general facility orientation
• Work alongside a case management team; no solo assignments
• Collaborate with social work team (15 full-time and 17 part-time social workers)
• Consult with Respiratory Therapy and Pharmacy as needed for discharge planning

Dress Code

• Any color scrubs or business professional attire

Notes

• The hospital is the only Level 1 trauma center in the state, handling complex patients and those without other hospital options
• Contracts with local facilities assist with complex patient placements under specific financial assistance criteria
