RN Case Manager - Hospice
Location: Lebanon, New Hampshire
Employment Type: Contract
Contract Length: 13 weeks

Job Description
- Provide pain management and comfort care for patients with terminal or chronic illness.
- Develop and periodically review patient care plans in collaboration with physicians, patients, and caregivers.
- Coordinate care through interdisciplinary communication and case conferences.
- Perform ongoing assessments of clinical, functional, and emotional status to identify problems or progress.
- Communicate significant changes to physicians and team members.
- Document evaluations, patient responses, and progress toward goals.
- Teach patients and caregivers care techniques and end-of-life symptom management.
- Work as part of a cohesive interdisciplinary team with broad independence.
- Maintain a work/life balance with flexible scheduling.
Required Qualifications
- Valid Registered Nurse (RN) license in New Hampshire.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Minimum of 2 years of hospice nursing experience, including hospice experience within the last year.
