Job Description

The Physical Therapist (PT) will provide physical therapy services to patients in their homes, following individualized plans of care. This role requires specialized knowledge, judgment, and skills in physical therapy, integrating psychological, biological, and social service principles. The PT will work under the VNA Clinical Team Manager, Rehabilitation, and adhere to organizational policies and procedures.

Minimum Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program approved by the American Physical Therapy Association (Master’s or Doctorate preferred)
• Current licensure and registration as a Physical Therapist in Massachusetts
• Current American Heart Association Basic Life Support (BLS) certification
• At least 1 year of physical therapy experience in a healthcare setting; home care experience preferred
• Strong knowledge of physical therapy principles
• Excellent verbal and written communication skills and interpersonal abilities
• Valid driver’s license, good driving record, and reliable transportation

Required Credentials and Compliance

• BLS Certification (AHA preferred)
• Professional license verification (no conditional licenses accepted)
• N95 Respirator Fit Test (must be completed prior to start and updated annually)
• Driver’s License/State ID
• MGB Badge Photo (professional headshot)
• Motor Vehicle Record (MVR) check
• Immunizations and Health Screenings:

  • MMR (Measles, Mumps, Rubella) vaccination or immunity proof
  • TB test (skin or blood test) within 3 months of start date
  • Influenza vaccine during flu season (Sept 15 - April 30)
  • Hepatitis B vaccination or immunity (strongly preferred)
  • Health statement signed by a licensed provider within 1 year
  • Tdap vaccine (preferred)
• Criminal background checks including:

  • MGB Criminal Background Check
  • Massachusetts CORI check
  • National Sex Offender Registry check
  • Exclusion checks against OIG, GSA, Medicaid exclusion lists
• Respirator Medical Clearance Attestation
• Education verification
• Social Security Number trace
• Submission of professional resume including 7 years of work experience
• References and skills checklist required for submission

Additional Notes

• No religious exemptions for N95 fit testing or influenza vaccine
• Fit testing cannot be performed onsite except in limited locations
• Documentation must be current and updated as required
• Compliance with all Mass General Brigham policies and procedures is mandatory
