Job Opportunity: RN Home Health (LTA)

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (approximately 13 weeks)

Job Summary

Provide clinical services to patients of all ages in their homes, including assessment, care plan implementation, and ongoing education and support. Responsibilities also include admissions and case management following organizational policies and standards, with thorough electronic documentation and interdisciplinary communication.

Required Qualifications

Current RN licensure in Massachusetts
Minimum 1 year of Certified Home Care experience
Minimum 1 year of Case Management experience
Reliable, independent transportation for home visits
Willingness to work weekends, holidays, and be on call as needed (weekend rotations occur once every 6 weeks)

Additional Information

Start date depends on credentialing clearance
