Description

RN Case Manager - Hospice

Location: Lebanon, New Hampshire
Employment Type: Contract
Contract Length: 13 weeks
Shift: Days
Hours per Week: 36

Job Description

Provide pain management and comfort care for patients with terminal or chronic illness.
Develop and periodically review patient care plans in collaboration with physicians, patients, and caregivers.
Coordinate care through interdisciplinary communication and case conferences.
Perform ongoing assessments of clinical, functional, and emotional status to identify problems or progress.
Communicate significant changes to physicians and team members.
Document evaluations, patient responses, and progress toward goals.
Teach patients and caregivers care techniques and end-of-life symptom management.
Work as part of a cohesive interdisciplinary team with broad independence.
Maintain a work/life balance with flexible scheduling.

Required Qualifications

Valid Registered Nurse (RN) license in New Hampshire.
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
Minimum of 2 years of hospice nursing experience, including hospice experience within the last year.
