RN Case Manager - Hospice

Location: Lebanon, New Hampshire

Job Description

  • Provide pain management and comfort care for patients with terminal or chronic illness.
  • Develop and periodically review patient care plans in collaboration with physicians, patients, and caregivers.
  • Coordinate care through interdisciplinary communication and case conferences.
  • Perform ongoing assessments of clinical, functional, and emotional status to identify problems or progress.
  • Communicate significant changes to physicians and team members.
  • Document evaluations, patient responses, and progress toward goals.
  • Teach patients and caregivers care techniques and end-of-life symptom management.
  • Work as part of a cohesive interdisciplinary team with broad independence.
  • Maintain a work/life balance with flexible scheduling.

Required Qualifications

  • Valid Registered Nurse (RN) license in New Hampshire.
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
  • Minimum of 2 years of hospice nursing experience, including hospice experience within the last year.
