Description

Job Title: Home Health LVN/LPN - Days

Join our team in Dorchester, MA, as a Home Health LVN/LPN for a 13-week contract, with potential extension. Work 40 hours per week on day shifts.

Responsibilities:
• Provide quality home health care to patients.
• Manage a daily caseload of 6 patients.
• Collaborate with healthcare teams for comprehensive care.
• Document accurately, HCHB experience preferred.

Qualifications:
• Valid MA LVN License.
• BLS Certification.
• 2+ years in Home Health.
• Strong communication and organization skills.

• No on-call duties.
• Rare floating to other locations.
