Description

We’re looking for an experienced Physical Therapist to join a supportive Home Health team in the Baltimore area. This is a hands-on clinical role focused on delivering exceptional patient care through evaluations, treatment plans, reassessments, and patient/family education.

✅ Requirements:

Active PT license
Home Health experience
OASIS knowledge

✨ Preferred:

HCHB (Point of Care) experience

📝 Details:

~30 units per week
Coverage: Baltimore City & County (with nearby areas as needed)

Collaborate with a dedicated interdisciplinary care team while working independently in the field
