Description
We’re seeking a motivated and compassionate Physical Therapist to join our growing home health team serving Montgomery County and Prince George’s County. This role is ideal for clinicians who are passionate about patient-centered care, independent practice, and helping patients regain mobility and confidence in their own homes.
Schedule:
Monday–Friday, 8:00 AM–5:00 PM (hours may vary based on patient needs; optional weekends by agreement with therapist/patient)
On-Call: Not required
Mileage Reimbursement:
$0.45 per mile
The Physical Therapist will conduct comprehensive evaluations, create individualized treatment plans, and deliver evidence-based care to improve patient outcomes. You’ll collaborate closely with physicians, nurses, and interdisciplinary team members to ensure continuity and quality of care across settings.
Primary Responsibilities:
- Perform thorough physical therapy evaluations and treatments based on physician orders/referrals.
- Develop and implement patient-specific care plans and measurable goals for functional improvement.
- Reassess patient progress regularly, modifying goals and treatment plans as needed.
- Provide education to patients and caregivers on mobility, exercise programs, and safety.
- Communicate effectively with the care team, physicians, and family members.
- Document care accurately and promptly in Homecare Homebase (HCHB) or OASIS systems.
- Recommend adaptive equipment and provide training for its use.
- Supervise Physical Therapist Assistants and Aides per state practice regulations.
- Maintain compliance with HIPAA, company policies, and professional standards of practice.
- Participate in staff meetings, continuing education, and development activities.
Requirements:
- Active Maryland or Compact State PT License (required)
- OASIS documentation experience (required)
- Homecare Homebase (HCHB) EMR experience (required)
- Reliable vehicle, valid driver’s license, and auto insurance
- Strong customer service, communication, and time management skills
Preferred Qualifications:
- Previous home health experience
- Experience supervising PTAs or aides
- Excellent interpersonal and organizational skills
