Registered Nurse (RN) -Care Coordinator

Core Role and Purpose

• Assess transitional care needs of patients and coordinate care across the continuum to ensure patient and family needs are met.
• Manage patients’ length of stay and effective resource utilization.
• Facilitate integration and coordination of care progression and transitional care planning.

Responsibilities

• Assess psychosocial and functional status, collaborate on discharge and care plans.
• Lead/facilitate patient/family care conferences and patient education.
• Collaborate with utilization review nurses, physicians, and interdisciplinary teams to evaluate medical, psychosocial, and social determinants of health.
• Monitor and support patients’ progress toward discharge goals, addressing barriers proactively.
• Consult with appropriate departments to resolve obstacles and expedite patient discharge.
• Participate in interdisciplinary rounds and develop post-acute referrals.
• Document assessments, care plans, and services accurately.

Professional and Administrative Duties

• Complete all ongoing competency assessments and required education.
• Act as a preceptor/mentor for student interns (where applicable).
• Comply with Wellstar policies, standards, and code of conduct.

Qualifications

• Associate’s Degree in Nursing or graduation from an accredited nursing school; RN license (Georgia or Multi-state Compact).
• Certifications: Current BLS (Basic Life Support).
• Minimum 1 year nursing experience in acute care settings.
• Skills: Strong communication, problem-solving, organization, ability to function under stress, collaborative teamwork, knowledge of regulations, and community resources.
