Job Title: LPN (Licensed Practical Nurse) Home Health
Location: Walla Walla, WA 99362
Shift: Days 5x8 Hour
Travel Pay Rate: $2200 weekly ($23.90/hr on w2 + $1244/week stipend)
Local Pay Rate :$1880 Weekly ($47 ON W2)

Position Summary:

The LPN Home Health Nurse provides skilled nursing care to patients in their homes under the supervision of a Registered Nurse or physician.
This role includes monitoring patient conditions, administering prescribed treatments, maintaining accurate documentation in Home Care Home Base (HCHB), and ensuring quality, compassionate care that supports patient recovery and independence.

Key Responsibilities:

Provide skilled nursing care to patients in their homes under RN or physician supervision.
Monitor and record vital signs, symptoms, and patient progress.
Communicate effectively with RNs, physicians, therapists, and other healthcare professionals.
Participate in interdisciplinary team meetings and case conferences.
Follow individualized care plans and update as directed by supervising RN.
Follow infection control and safety protocols at all times.
Maintain patient confidentiality in compliance with HIPAA regulations.

Required Qualifications:

BLS
LPN license in hand
2 Years Of Home Health Experience
