Required Skills & Qualifications:
- Current licensure in a clinical discipline per state guidelines (RN, RT, SW preferred)
- Associate’s Degree in a healthcare field (Bachelor’s preferred)
- Working knowledge of insurance industry and government reimbursement
- 2 years of Case Management and Discharge Planning experience in a Rehab Care setting

📝 Description:
Short Description:
The Case Manager is responsible for utilization reviews, resource management, discharge planning, treatment plan management, and financial management, while also completing medical record documentation.

Key Responsibilities:
- Follow facility guidelines for appearance, attendance, and punctuality.
- Identify patient or family problems affecting treatment participation.
- Assist patients and families in utilizing hospital resources to adjust to healthcare situations.
- Collaborate with the interdisciplinary team to develop and manage clinical care and discharge plans.
- Interact appropriately with patients according to age and understanding.
- Refer patients and families to outside agencies as needed.
- Inform DCM/CEO of major conditions, changes, and emergencies.
- Complete and submit accurate documentation and statistical reports.
- Conduct team and family conferences per policy.
- Ensure utilization review criteria are implemented for all admissions and stays.
- Maintain community resource systems and assist families in accessing services.
- Ensure clinical documentation meets payor guidelines for reimbursement.
- Work with clinical teams to identify unnecessary procedures.
- Complete discharge planning evaluations per policy and regulations.
- Maintain fiscal responsibility and ensure proper billing levels.
- Perform Director of Case Management duties in their absence.
- Manage payor communications, certifications, and satisfaction.
- Document plans of care and progress notes per policy.
- Perform other duties as assigned.
