Description

Role Overview:
The Case Management Manager serves as the Team Lead for the Case Management Department, directing daily functions of the Case Management staff, including providing input on job evaluations and work assignments.

Responsibilities:

Ensure all Case Management functions occur regularly and are documented appropriately.
Oversee communication with third-party payers and financial services.
Serve as the second layer of review for discrepancies in levels of care identified by Case Management staff.
Escalate unresolved issues to the CNO when needed.
Oversee patient care coordination, resource utilization, and discharge planning.
Lead Case Management staff and collaborate with medical staff, insurance providers, and community resources.

Required License:
RN license required.
