Requirements
- Active LPN license in good standing
- Minimum 1 year of Long-Term Care (LTC) experience
- Experience with PointClickCare (PCC) charting system required
- Ability to provide 2 professional references
- Strong medication administration and resident care skills
- Ability to work independently and as part of a care team
- Dependable transportation to facility
Responsibilities
- Provide direct resident care in a long-term care setting
- Administer medications and treatments per care plans and RN direction
- Accurately document care in PCC (PointClickCare)
- Monitor and report changes in resident condition
- Assist with ADLs and support CNA staff as needed
- Follow facility policies, safety protocols, and state regulations
- Promote dignity, comfort, and quality of life for residents
