Schedule: 5x8s

Requirements

Active RN license
BLS certification required
Minimum 2 years of acute hospital Case Manager experience
Experience in care coordination & discharge planning
Epic charting experience required
Must have recent work history within the last 2 years

Required Skills & Experience

Care coordination & plan of care development
Discharge planning & transitions of care
Medical necessity determination
Prior authorizations
DME, SNF, and ambulance transport coordination (independent)
Utilization Review & InterQual criteria (preferred)
Strong interdisciplinary communication
