Assignment Details

Duration: 13 weeks
Shift: Days – 8:00 AM to 4:30 PM
Schedule: 5x8s

Requirements

Active RN license
Minimum 2 years of acute hospital Case Management experience
Experience in care coordination and discharge planning
Epic charting experience required
Current acute care case management experience within the last year (HARDSTOP)

Required Skills & Experience

Care coordination & plan of care development
Discharge planning & transitions of care
Benefits eligibility & prior authorizations
DME, SNF, and ambulance transport coordination
Experience with HMOs, IPAs
Advance directives, POLST, and protective services processes
Strong interdisciplinary communication

Current acute care case management experience within the last year (HARDSTOP)
