Description
The RN Case Manager - Hospice:
Develops a care plan in collaboration with the patient's physician, the patient, and their family, or the primary caregivers upon admission. Periodically reviews the patient care plan and recertifies the patient care plan as appropriate.
Coordinates the patient's care plan through interdisciplinary communication and case conferences, as appropriate.
Provides ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and/or progress toward desired goals. Communicate significant changes to the patient's physician and VNH colleagues.
Documents evaluations, patient response, and progress towards goals.
Teaches the patient and caregiver care techniques and end-of-life symptom management as appropriate.

I have worked with this company for 3, along with a few others, and they have been great! They seem to be very appreciative of their staff and always sending appreciative gifts to you. I can’t say much now that they have joined TotalMed, but am looking forward to see what develops.
Cons
Rates are not always the best, would be nice to have a few more eastern and southern state options, like Mississippi or North Carolina cities
