Physical Therapist - Home Health (Travel/Contract)

Location: Dublin, Ohio
Contract Length: 13 weeks

Job Details

• Specialty: Home Health Physical Therapy
• Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 5:00 PM)
• Guaranteed 40 hours per week
• Average caseload: 5-7 patients per day (geriatric population)
• Territory: Up to 60-mile radius from the office hub
• On-call required every 6th weekend
• Dress code: Scrubs of any color; jeans not allowed
• iPad provided for documentation; cell phone use not allowed during work
• Mileage reimbursed at IRS standard rate
• Flexibility with territory expected
• Timely documentation and communication required
• First-time travel therapists not accepted (prior travel experience required)

Required Qualifications

• Valid Physical Therapist license in Ohio
• Minimum 1 year of Physical Therapist experience within the last year
• Basic Life Support (BLS) certification
• Experience with Homecare Homebase (HCHB) electronic medical record system

Background and Compliance Requirements

• Comprehensive background checks required within 90 days of start, including:

  • SSN trace, county searches (last 7 years), FACIS Level 1, nationwide sex offender, motor vehicle records, education and work history verification
  • State-specific searches as applicable (e.g., Ohio BCI and FBI fingerprinting updated annually)
• Fingerprinting required and must be current within 12 months; reprints required if candidate has lived/worked outside Ohio since last prints
• Education verification of highest degree related to profession
• Signed acknowledgment of Orientation Safety Manual (updated annually)
• NURSYS verification as applicable
• Competency exams including medication/pharmacology test as applicable
• Proof of current auto insurance required due to driving responsibilities

Health and Immunization Requirements

• Physical exam within the last 12 months (not required annually)
• Negative TB test (PPD, Q Gold, or TSpot) within 1 year or chest X-ray within 5 years if positive PPD
• Annual TB questionnaire required
• Immunizations or proof of immunity for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• 9-panel drug screen within 12 months (instant acceptable)
• Seasonal flu vaccination or facility-specific declination form required during flu season
• COVID vaccination or facility-specific declination required

Documentation and History

• 7 years of full work history with no gaps, including city and state of employment
• Employment history verification within 90 days of start (3 attempts required if unverifiable)
• Two professional references required within 2 years of start (travel, allied, per diem acute care)

Additional Notes

• Locals are not accepted for this travel contract
• Candidates must comply with facility-specific policies including Acceptable Use Policy and flu/COVID statements
• Personal or auto insurance proof must be maintained by the agency but not submitted to the facility
