Job Description
- Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits with an average of 5-8 patients per day.
- Types of visits include Start of Care, Wound Care, Follow-up Visits, etc.
- Must have reliable transportation, current driver's license, and agency-required liability insurance.
- Willingness to travel up to 75 miles from home clinic; approx. 50-100 miles traveled weekly.
- Use of electronic medical records (EMR) systems: HomeCare HomeBase (HCHB), Oasis, and PointCare.
- Dress code: Scrubs.
Requirements
- Minimum 1 full year of recent Adult Home Health or Hospice experience (depending on specialty).
- First-time travelers not accepted; recent specialty experience required.
- Certifications: Current CPR and BLS required.
- Must have an active nursing license (no pending licenses accepted).
- Proficiency with HomeCare HomeBase (HCHB) within the last 6 months required; retraining provided if last use was over 6 months ago.
- RNs must be proficient in Oasis documentation.
- Local candidates accepted at the same rate.
Productivity Expectations
- Weekly productivity goal: 30 points (points assigned based on visit type and activities).
- Typically assigned 5-10 patients per day equating to roughly 6-8 points daily.
Territory
- Coverage area: Northeast/Southeast Washington DC region.
- Must be willing to travel up to 75 miles from the home clinic location.
Compliance & Documentation Requirements
- Must provide a valid driver’s license/state ID.
- Personal or auto insurance required for home health placements; policy must be current and under candidate’s name or list candidate as a driver.
- Must complete and sign various compliance documents prior to start and annually if working more than one year consecutively, including but not limited to:
- NDA and End User Agreement
- Compliance Basics and Practice Quiz
- Code of Conduct
- HIPAA and Privacy Policy
- Confidentiality Form and Covenant
- Home Health Core Policy Acknowledgement and Scenarios
- COVID-19 Policy and Attestation
- Employee Responsibility Agreement
- Hepatitis B Form
- TB Questionnaire
- Background Check (7 years, including SS trace, county, nationwide, sex offender, and state-specific searches)
- Drug Testing (10-panel)
- Physical Exam within 12 months
- Immunizations and Titers: Measles, Mumps, Rubella, Hepatitis B, Varicella
- Flu and COVID-19 vaccination or documented declination (DC state policy requires vaccination without declination)
- Fingerprinting as required by state and facility policies
- Work and education verification
Additional Notes
- Contract assignments typically last 13 weeks.
- No weekend or on-call requirements for this position.
- Workload growth is driving the need for this role.
- Candidates must be proficient in electronic charting and documentation systems used by Amedisys.
- Mileage is reimbursed at the IRS standard rate for travel between patient homes (commute to/from first and last patient is not billable).
This position is ideal for experienced Registered Nurses with recent home health experience who are comfortable with travel within the DC area and proficient in home health documentation systems.
