• Assignment location may vary between hospitals based on daily need

Job Description

• Provide direct clinical social work services including counseling, discharge planning, and case management for hospital patients and their families
• Assess and document emotional, social, financial, environmental, and follow-up referral needs
• Collaborate with interdisciplinary healthcare team to develop and implement psychosocial plans of care tailored to patient strengths and needs
• Develop and monitor discharge plans and progress toward goals of care, identifying barriers to meeting estimated length of stay
• Facilitate integration with community-based resources to optimize patient health
• Provide psychological assessment and support across acute, long-term, and community health settings
• Support quality improvement, clinical research, and community networking efforts
• Complete required documentation in patient medical records and electronic systems
• Advocate for patients and families through integrated mind-body-spirit care approaches
• Maintain constructive relationships with team members, external providers, and management

Required Qualifications

• Master of Social Work (MSW) from a program accredited by the Council on Social Work Education
• Licensure preferred but not required
• Strong psychosocial assessment and critical thinking skills with ability to exercise independent clinical judgment
• Skilled in working with patients and families coping with loss and grief
• Knowledge of motivational, system, developmental theories, and group dynamics
• Ability to work effectively with diverse populations across age, culture, ethnicity, and gender
• Strong organizational skills and attention to detail
• Proficiency with word processing; database skills strongly preferred
• Excellent written and oral communication skills
• Effective caregiving and advocacy skills for clients ranging from neonatal through geriatric
• Ability to build and maintain collaborative relationships within a multidisciplinary team
• Demonstrated respect for diverse perspectives

Physical Requirements

• Some standing, walking, stooping, bending, twisting, and sitting with computer use throughout the workday
• Transportation available to cover referrals when needed
