Licensed Practical Nurse (LPN) - Medical Surgical (Travel/Contract)

Location: Rochester, New York
Contract Length: 13 weeks
Shift: Day shift, 4x10 hours (10:00 AM - 7:00 PM), 4 days per week (must include Thursday and Friday)
Weekend Requirement: Every other weekend
Holiday Requirement: Available to work every other holiday, if needed


Job Description

  • Work in a discharge lounge setting caring for medical patients with conditions such as COPD exacerbation, infections, failure to thrive, and social issues.
  • Patient ratio approximately 1:5-6 during day shifts.
  • Must be able to take own patient assignment with RN support.
  • Floating within skillset may be required.
  • Charting experience with EPIC required.
  • Scrub color: navy blue and white combinations accepted.

Required Qualifications

  • Valid LPN / LVN license in New York (NY is not a compact state).
  • Minimum of 1 year Long Term Care experience within the last year.
  • Med Surg experience.
  • Basic Life Support (BLS) certification issued by the American Heart Association.
  • Experience with Epic electronic health record system.

Scheduling Details

  • Must submit time off and special scheduling requests upon contract submission.
  • The client may cancel up to three shifts during the 13-week contract without charges.
  • First-time travelers are accepted.
  • Local candidates accepted with adjusted pay rate.

Required Onboarding and Compliance

  • Government-issued photo ID required.
  • 7-year criminal background check within 3 months of start (including felony, SSN trace, national criminal, sex offender, education verification).
  • EPLS/GSA/SAM and HHS/OIG checks required within 3 months of start and monthly thereafter.
  • Foreign education equivalency evaluation if applicable.
  • Education verification from primary source required (not applicable for CNA).
  • Primary source verification of state license within 30 days of start.
  • NURSYS verification for LPN license within 30 days of start.
  • Medication/pharmacology competency exam as required.
  • Color blindness testing required once.
  • Competency exams as required by specialty.
  • Orientation safety manual acknowledgement required.
  • Facility-specific documents including photo for badge and parking pass form required.
  • Physical exam within 1 year of start, updated annually.
  • Fit mask test within 1 year of start, updated annually (specific mask types accepted).
  • Negative TB test or chest X-ray within 1 year of start; TB questionnaire accepted in lieu of annual renewal.
  • Immunizations or titers required for:
    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B
    • Varicella
  • Drug screening within 30 days of start (includes amphetamines, cocaine, fentanyl, heroin, opiates, etc.).
  • Seasonal flu vaccination or signed declination required during flu season.
  • Snellen eye exam within 1 year of start.
  • COVID vaccination copy preferred but not required.

Additional Requirements

  • Employment application and skills checklist required annually.
  • Work history verification required (at least two past employments).
  • References required for some agencies (2 references within 3 years).
  • State-specific requirements for New York including:
    • Nurse Aide Registry (if applicable).
    • Criminal background fingerprinting and disciplinary action searches for certain units.
    • Sex offender registry search within 30 days of start.
    • Medicaid exclusion list check within 30 days of start and monthly thereafter.

Notes

  • Must be available to work every other weekend and every other holiday if needed.
  • Candidate must complete all health-related requirements and lab work within 5 days of contract signing.
  • Floating may be required within the skillset.
  • EPIC charting experience is mandatory.
  • First-time travel nurses are welcome.
