Physical Therapist (PT) Job Opportunity

Location: Waco, Texas
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Job Description

• Perform initial and ongoing assessments of patients' conditions.
• Establish, revise, and evaluate individualized plans of care involving patients and their families.
• Deliver therapy interventions using standard physical therapy techniques appropriate to patient conditions.
• Supervise physical therapy assistants, students, and support personnel.
• Maintain targeted productivity levels.
• Assist at other sites as patient demand requires.

Required Credentials & Compliance for Onboarding

Professional Credentials (Required for Submission)

• Resume/Profile
• Driver's License or State ID (valid, not expired, matching address requirements)
• At least two professional references

Health & Personal History Requirements (Required for Onboarding)

• MMR: Documented proof of two MMR vaccines or positive IGG titer (no declinations accepted). If negative titer, two doses of MMR vaccine four weeks apart required.
• Fit Test: Required for all travelers; preferred before start but can be completed onsite (annual update required).
• Hepatitis B: Documentation of vaccination series completion or signed declination.
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement.
• 2 Step PPD or IGRA (Quantiferon/Tspot): 1st PPD within 1 year, 2nd within 30 days of start, or negative IGRA within 90 days of start. Annual TB questionnaire required.
• Varicella: Proof of two vaccines, serological immunity, or verified history; no declinations accepted.
• Tdap: Valid within 10 years; no declinations accepted.
• Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually.
• Ascension 12 Panel Drug Screen: Within 30 days prior to start, updated annually; retesting required if break in service > 90 days.

Professional Credentials (Required for Onboarding)

• Criminal Background Check: Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender, and other checks; updated annually or after break in service > 90 days.
• EPLS (Excluded Parties List System) Check: Run within 90 days of start; rerun if break in service > 90 days.
• Skills Checklist: Clinical checklist to be completed within one year of start and updated annually.
• Education Verification: Verification of education relevant to clinical work within 90 days of start; rerun if break in service > 90 days.
• Confirmation of Assignment: Provided upon initial assignment and updated with each extension.
• Employment Verification: Third-party verification for past seven years; updated annually or after break in service > 90 days.
