Required Certifications & Licenses

• BLS (Basic Life Support): AHA, Red Cross, CHSF, or Military certification accepted.

• Tdap Vaccine: Preferred within last 10 years and updated upon expiration.
• Fit Test: N95 respirator fit test within 365 days of start date; updated annually. No religious exemptions.

Background and Compliance Checks

• Criminal Background Check: Must be completed within 45 days prior to start date, including all counties resided, worked, or attended school in the last 7 years.
• Massachusetts CORI Check: Required within 30 days prior to start date.
• SSN Trace: Must list addresses/aliases for past 7 years.
• National Sex Offender Registry Check: Conducted within 30 days of start date and updated annually.
• Exclusion Checks: Search of OIG, GSA, MassHealth, and NH Medicaid exclusion lists; attestation required.
