Vaccination and Health Requirements

- MMR (Measles, Mumps, Rubella): Documentation of two doses of each vaccine or two MMR vaccines, or proof of immunity via blood test.
- Hepatitis B: Strongly preferred to have 3 doses or proof of immunity or declination.
- Influenza Vaccine: Required during flu season (Sept 15 - April 30). No exemptions or waivers allowed.
- TB Test: Documentation of TB skin test or negative QuantiFERON/T-spot blood test within 3 months of start date; must be updated if there is a break >6 months.
- Health Statement: Physician statement signed by MD/DO/NP/PA within 365 days of start date; updated if break >6 months.
- Tdap Vaccine: Preferred within last 10 years and updated upon expiration.
- Fit Test: N95 respirator fit test within 365 days of start date; updated annually. No religious exemptions.
Background and Compliance Checks
- Criminal Background Check: Must be completed within 45 days prior to start date, including all counties resided, worked, or attended school in the last 7 years.
- Massachusetts CORI Check: Required within 30 days prior to start date.
- SSN Trace: Must list addresses/aliases for past 7 years.
- National Sex Offender Registry Check: Conducted within 30 days of start date and updated annually.
- Exclusion Checks: Search of OIG, GSA, MassHealth, and NH Medicaid exclusion lists; attestation required.

- References: Two recent references required, at least one supervisor or above.
- Skills Checklist: Required for submission.
- Medication Exam: 80% passing score within 365 days for RN & LPN.
