Minimum Requirements
- Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
- Active State License is Required to Start the Assignment
- BLS Certification May Be Required from AHA or ARC
- Home Health Experience
- Physical Therapist License in MI
- Minimum Education Requirement: Doctoral
Assignment Details
- Shift Hours: 8:00 AM - 4:30 PM.
- Shift Days: Monday through Friday.
- No on-call requirement.
- Coverage area includes Marquette County and the western portion of Alger County within a 1-hour radius of Marquette.
- Mileage is reimbursed between visits, from first patient to last patient.
- Responsibilities include physical therapy admissions, add-on evaluations, reassessments, discharges, creating individualized plans of care, supervising and working in conjunction with PTA, and participating in weekly case conferences and monthly staff meetings.
