Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Active Physical Therapist license in WV required to start
• New graduates are not accepted
• HEP B required
• TB required
• Tdap required

Assignment Details

• Two evening shifts until 7:00 PM
• Fridays until 4:00 PM
• Clinic hours are Monday through Thursday, 7:00 AM to 7:00 PM, and Friday, 7:00 AM to 4:00 PM
• Floating required between clinic locations with a set schedule
• Documentation system: NextGen
