Job Summary

We are seeking a dedicated Physical Therapist to provide physical therapy services to patients in their homes. The ideal candidate will work with individuals experiencing physical impairments, functional limitations, or disabilities due to various causes, including injury or illness. Patients may range in age from birth to over 100 years.

Key Responsibilities

• Deliver personalized physical therapy services at the patient's residence.
• Collaborate with patients, families, physicians, and other healthcare professionals to assess patient needs and coordinate appropriate services.
• Ensure timely and thorough electronic documentation in accordance with agency policies and standards.
• Communicate interdisciplinary information effectively as needed.
• Travel to assigned cases as necessary.
• Participate in weekend rotations approximately once every six weeks.

Qualifications

• Active Physical Therapist Professional license in Massachusetts.
• Minimum of one year of experience in home care.
• Minimum of one year of case management experience within a certified home health agency.
• Experience in hospice or private care will not be considered relevant for this role.

Additional Information

The start date for this position will be determined based on credentialing clearance. If the credentialing process is not completed by the planned start date, a new start date will be scheduled.
