Description

Home Care Registered Nurse (RN)

Position Overview

This is a full-time Home Care RN position responsible for delivering individualized, goal-oriented nursing care in accordance with physician orders. The role involves utilizing the nursing process to assess, plan, implement, and evaluate patient care. Additionally, this position includes case management responsibilities when serving as the lead discipline in patient care.

Key Responsibilities

Provide comprehensive nursing care to patients within their homes.
Develop and implement individualized care plans based on each patient's unique needs.
Coordinate with healthcare professionals to deliver seamless patient care.
Serve as a case manager for patients, overseeing their care and ensuring effective communication with other healthcare providers.
Monitor patient progress and adjust care plans as necessary.
Educate patients and families on health management and wellness practices.

Qualifications

Minimum of 1 year of nursing experience required.
Current BLS/BCLS certification is required.

Work Schedule

This position follows a Monday to Friday schedule with weekend rotation approximately every fourth week. Holiday coverage may be required.
