Home Health Physical Therapist Summary:

• Review and interpret physician therapy orders for patients in Home Health and Hospice settings.
• Conduct comprehensive patient evaluations and establish written treatment plans with specific short-term and long-term goals.
• Provide skilled physical therapy interventions, including therapeutic exercises, stretching, mobility training, balance activities, pain management techniques, and functional rehabilitation.
• Evaluate home safety and recommend appropriate equipment, assistive devices, or modifications to the environment to enhance patient independence and prevent falls.
• Monitor patient progress, making modifications to treatment plans as needed to ensure optimal outcomes.
• Educate patients, caregivers, and family members about treatment plans, home exercise programs, safety measures, and expected recovery outcomes.
• Maintain precise and timely clinical documentation, including evaluations, progress notes, discharge summaries, recertifications, resumptions of care, and discharge documentation.
• Collaborate and communicate effectively with physicians, nurses, therapists, and other healthcare professionals involved in patient care.
• Travel to patient residences and various care settings to deliver therapy services.

Requirements:

• Current physical therapy licensure in the state of Montana.
• Valid driver's license.
• BLS Certification.
• A minimum of 2 years of relevant experience.

Work Schedule:

• 36 hours per week - Day shift.
• Holiday coverage required.
