Description
The Case Manager Registered Nurse (RN) plays a vital role in providing high-quality hospice care. This position primarily works Monday through Friday with a rotating weekend schedule every third weekend.
Position Information
- Role: Registered Nurse
- Specialty: Hospice
- Shift: 0800-1700
- Weekly Guaranteed Hours: 40
- Weekend Rotation: Every third weekend; includes on-call rotation overnight
- Productivity Expectations: 25 visits per week
- On-Call Requirements:
- Weekday on-call from 5PM to 8AM, with rare prn visits overnight
- Weekend on-call from Friday at 5PM through Monday at 8AM
- Estimated on-call frequency: 1x per week and 1 weekend per month
Qualifications
- Years of Experience: Minimum of 1 year in home health or hospice settings
- Required Certifications: BLS
- Preferred Skills/Experience:
- PICC lines, dressing changes, tracheostomy care, port access, ostomy management, chest tubes, wound care, lab draws, and catheters
- Patient Types: Adult medical surgical, oncology, cardiac, neurological, respiratory, gastrointestinal, and genitourinary patients
- Personality Traits: Collaborative mindset, strong communication skills, openness to feedback and practice improvement
Unit Information
- Patient Ratios: Manage 14-18 patients based on acuity, expected to complete 5-6 visits each day
- Equipment Provided: Agency laptop, iPhone, nursing supply bin, nursing bag with vital signs equipment, and agency bag
- EMR System: EPIC
Dress Code
- Scrub Color: Business casual or scrubs of any color
- Unacceptable Attire: No jeans, open-toed shoes, or clothing with holes
Additional Information
Candidates must be capable of effectively managing and self-scheduling their patient visits. Experience with in-home infusion pumps is required, while ventilators and invasive monitoring will not be part of the responsibilities.
