Job Description

We are seeking a dedicated and skilled Pharmacist to join our team at a 94-bed hospital. The ideal candidate will play a crucial role in the medication management process, ensuring the safe and effective use of pharmaceuticals within the healthcare setting.

Key Responsibilities

• Contribute to patient care by providing accurate medication information and ensuring appropriate patient medication therapy management.
• Collaborate with healthcare professionals to optimize treatment plans based on clinical evidence.
• Review and verify medication orders for accuracy and appropriateness.
• Monitor patient responses to medications, conducting follow-up as necessary.
• Educate patients and their families on medication use, potential side effects, and interactions.
• Participate in quality improvement initiatives and maintain compliance with regulatory standards.
• Provide consultation and recommendations to healthcare providers regarding drug therapy.

Shift Information

This position requires flexibility to work days, evenings, and weekends, including holiday coverage as necessary.

Qualifications

• Minimum of 2 years of experience as a Pharmacist.
• Current certification in Basic Life Support (BLS) or Basic Cardiac Life Support (BCLS) is required.
• Valid pharmacist license in the state of practice.
