Job Summary

We are seeking a dedicated Physical Therapist to provide therapy services to patients in their homes. Candidates must have a valid Physical Therapist Professional license in Massachusetts and at least one year of experience in home care. The therapist will work with patients of all ages who have physical impairments, functional limitations, or disabilities arising from various causes, including injury and disease.

Key Responsibilities

• Deliver high-quality physical therapy services to patients in their residences.
• Collaborate with patients, families, physicians, and other healthcare professionals to assess patient needs.
• Develop and implement individualized treatment plans to meet patient goals.
• Utilize electronic documentation for thorough and timely patient records.
• Ensure effective communication with an interdisciplinary team in accordance with agency policies and standards.

Qualifications

• Current Physical Therapist Professional license in Massachusetts is required.
• A minimum of one year of experience in home care is required.
• Ability to travel to different patient cases as assigned.
• Willingness to participate in weekend rotations once every six weeks.

Work Environment

The role involves traveling to various locations within the region, providing necessary therapy and developing strong relationships with patients and their families.
